Home Health Care for Joint Replacement Patients
in the Chicago Area

We often care for patients that have just undergone hip or knee replacement surgery. Though joint replacement will eventually improve the quality of life for patients, using a new joint for everyday tasks can be difficult for patients at first.

During every visit, joint replacement patients receive education that aids their recovery and best fits their learning style. Northwest Home Health & Rehab services may include:
  • Exercise and Physical Therapy
  • Safety Precautions
  • Incision Care
  • Pain Management
  • DME Implementation and Coordination
  • Nutrition Counseling
We work closely with nationally recognized orthopedic surgeons in the Chicagoland area to implement their orders regarding durable medical equipment (DME). Northwest Home Health & Rehab will contact the DME agency to place the order, and will work with the company to verify the patient’s insurance and benefits. Our intake coordinator will follow up with the DME agency to ensure the items have been delivered to the patient in a timely manner. Our care team works closely with each patient to help them regain independence after total joint replacement surgery. Proper care during recovery helps extend the life of a joint replacement surgery, improving movement, strengthening muscles and lessening pain for years to come.
