Telehealth Monitoring for Home Health Care Patients

Telehealth monitoring is a way for our home health care providers to check vital patient information in between care visits, including blood pressure, pulse, weight, oxygen saturation and more. 

We utilize the Honeywell HomMed system to check in at specified times, particularly with patients that have:
  • Congestive Heart Failure (CHF)
  • Hypertension
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Coronary Artery Disease (CAD)
The Honeywell HomMed telehealth monitoring system includes a pulse monitor, blood pressure monitor and scale to measure weight. Each system is customized to the individual patient by their case manager and programmed with up to ten subjective individual questions to monitor their health. Each check-in usually takes less than 3 minutes for patients to complete and is not disruptive to their recovery process. Through telehealth monitoring, we can offer 24/7 patient management and identify any trends in their vital signs. This helps prevent readmission to the hospital by quickly recognizing issues that may arise during recovery.
